LEV Documentation
Please save and complete this smart PDF, ideally before the engineer arrives, then return it to orders@pjdust.co.uk or bookings@pjdust.co.uk quoting your sales order number. Only one document is required per process.
For example, if you use multiple pieces of woodworking equipment, you only need to complete one form for the woodworking process.
If possible, please email the following documents to orders@pjdust.co.uk or bookings@pjdust.co.uk before the engineer's visit together with your last LEV report if not carried out by P&J:

- Your DSEAR Risk Assessment for the process(es)
- LEV System Manual (if not a P&J system)
- SDS Safety Data Sheets for substances in use. Please note that the substances used should be referenced in detail in your COSHH Regulation 6 process method statement and risk assessment; if SDS are submitted to us separately from your risk assessment P&J will review a maximum of two different SDS sheets, please submit one that you have determined is the most hazardous substance in use and one that is the substance used in the greatest quantity.

This list is LEV Documentation that the HSE expect you to have, the engineer will ask if you have these, we do not need these emailed to us:

- LEV Commissioning reports (if carried out by P&J we will have records)
- The previous LEV reports (if carried out by P&J we will have records)
- LEV system log-book, records of user checks and maintenance
- Your COSHH risk assessment for the process(es)
- Summary of any work place air quality monitoring
